Step 1: Complete the Diversity Monitoring Form (a Google Form)

Once you have submitted the Diversity Monitoring Form, you will receive a uniquely-generated code (the format is ###-#####-#####-##-#######) at the post-submission page. Copy & paste that code into the signup form in Step 2.

Step 3: Check your emails!

An email should have been sent to you to confirm that you have successful signed up. Et voilà, welcome to the STRIVE Family!
